CBD After 60: What Changes
The Real Issue Is Not Age. It Is the Medication List.
Most people over 60 who consider CBD are already taking something else — often several things. That, far more than age itself, is what changes the picture.
CBD interferes with the liver enzymes that process a large share of prescription medicines, which means it can raise the effective strength of drugs already in use. The longer the medication list, the higher the chance one of them is affected. Our guide to CBD drug interactions covers which categories matter, and it is the single most important thing to read before starting.
Why the Starting Dose Should Be Lower
Liver and kidney function decline gradually with age, so a substance clears more slowly. Body composition also shifts, which affects how a fat-soluble compound like CBD distributes and how long it lingers.
The practical consequence is that the same dose does more. Starting at half what a younger person might start with, and increasing more slowly, is not excessive caution — it is matching the dose to the body.
Sedation Is a Fall Risk
Drowsiness is a common CBD side effect and usually a minor inconvenience. After 60, and particularly after 70, it is not minor: it contributes to falls, and a fall is often the event that changes everything.
This matters most if CBD is added to anything else sedating — a sleeping tablet, a benzodiazepine, an antihistamine, an opioid. The effects add up. If you are starting CBD, start in the evening at home rather than before a day out, and pay attention to balance in the first week.
What It May Genuinely Help With
Being honest about limits does not mean being dismissive. The reasons older patients most often try CBD are sleep quality, joint discomfort, and anxiety — and the evidence differs across all three. Our pieces on inflammation and anxiety set out what is actually supported.
What CBD offers some older patients is a comparatively well-tolerated option to try alongside existing care, with a side-effect profile that is milder than several alternatives commonly prescribed for the same complaints. That is a reasonable thing to explore. It is not a reason to expect transformation.
The Conversation Worth Having
Bring the full list — prescriptions, over-the-counter items, supplements, and the CBD product with its strength. Ask specifically about interactions, about whether monitoring is sensible, and about what improvement would look like so you can tell whether it happened.
Any physician registered with the Colegio de Médicos y Cirujanos can have this conversation and prescribe if appropriate. If your intended dose exceeds 70 mg a day, a prescription is required in any case — see when CBD needs a prescription in Costa Rica.
One Thing Not to Do
Do not reduce or stop a prescribed medication in order to make room for CBD. If the goal is to come off something, that is a supervised process with a taper, not a substitution you arrange yourself.
